We ask that you complete all forms prior to scheduling.

✔ Mental health & medical history form (Required)
✔ ID & insurance card upload (Required)
✔ Credit card authorization (Required)
✔ Signed office policies (Required)

STEP 3

SCHEDULE VISIT

Once your forms are complete, our team will review them and reach out to schedule your first appointment. If anything is missing, we’ll let you know!

Appointment times are not held until all documents are returned.

STEP 4

STAY INFORMED

You will receive a text and email reminders 4 days and 90 minutes before your appointment. If it’s a virtual visit, ensure you have a quiet, private space and stable internet connection. 

Please cancel your appointment with 24 hours notice to avoid fees.
